DCHS Medical Foundation Announces New Affiliations

San Jose, CA (July 19, 2013) – The Daughters of Charity Health System (DCHS) Medical
Foundation acquired the assets of Samaritan Family Practice in San Jose on July 1, 2013. The
medical group comprises five physicians trained to provide primary health care and services,
including routine adult and child physical exams, newborn guidance, gynecological preventive
care, and podiatric care. Members of the Samaritan Family Practice include: Jennifer D.
Haggerty, MD; Joceliza G. Chaudhary, MD; Manjula Kamaraju, MD; Norman P. Woods, MD;
and William S. Warshal, MD. The group has been providing complete and convenient health
care to South Bay residents for more than forty years.

Additionally, the Almaden Family Physicians Medical Group, Inc, recently joined the ranks of
the DCHS Medical Foundation. The seven member team is a group of Board Certified Family
Physicians specializing in Family Medicine including the treatment of both acute and chronic
illnesses, as well as preventative care of healthy patients of all age groups. They are also located
in South San Jose. Members of the Almaden Family Physicians Medical Group include: David
W. McCullough, MD; David L. Griffith, MD; John M. Wortley, MD; Jeffrey C. Ettinger, MD;
Henry Chua, MD; Manika Kaushal, MD; and Liangxue (Julia) Zhu, MD.

and experience and will join us as we change the way health care is delivered. We can continue

those who come to us for care.”

Also partnering with the DCHS Medical Foundation are Sheetal Ankolekar, MD, in Gilroy, and
Thang Nguyen , MD, in Daly City. Both physicians are family practitioners.

The DCHS Medical Foundation, a statewide medical foundation established by Daughters of
Charity Health System and headquartered in San Jose, CA, was formed in April 2012. Currently,
the DCHS Medical Foundation is expanding to serve DCHS communities statewide in
collaboration with local medical staffs at DCHS hospitals which span the California coast from
the San Francisco Bay Area to Los Angeles.

According to DCHS Medical Foundation CEO Ernie Wallerstein, the new affiliates will benefit
from joining the Foundation. “Physicians who are joining the DCHS Medical Foundation at this
time have the opportunity to create the organization from the ground up. They have a voice in
how we will accomplish our shared goals of achieving integrated care and successfully meet the
needs of the ever-changing health care environment.”
